We are happy to report that our office is virtually paperless.  Please compete our online intake paperwork and be sure to arrive 10-15 minutes early for your first visit.

If you are suffering from a work or auto injury, please be sure to notify us and be prepared to provide the necessary information.

Please also ensure that you bring your insurance card and personal identification (drivers license or passport) to your first visit.
